    CYCOLOGY CYCLING CLUB RIDE PARTICIPATION/ACTIVITIES WAIVER AND RELEASE FORM

    Please note that by participating in a Cycology Club Ride you hereby agree and acknowledge the following:

    That cycling involves certain inherent dangers, including but not limited to the hazards of roads and off-road terrain; weather; accidents; and actions of participants, motorists, and other persons. You acknowledge that you are aware of the rules of the road applicable to bicycles.

    That certified bicycle helmets are required to participate in all rides, and you agree to wear a helmet while participating in any ride and to follow the rules of the road and all applicable laws and safe cycling practices.

    That you understand that it is not the function of the ride leaders (if a ride has ride leaders) to serve as guardians of my safety.You recognize that the route(s) chosen may be challenging, not necessarily the safest or easiest route(s), and that weather, road, and traffic conditions may make a ride difficult.

    That you give permission to Cycology Cycling Club to use or release your image without payment should it appear in photos or videos taken during any ride.

    That you understand and agree that Cycology Cycling Club will not be held liable in any way for any occurrence or accident in connection with a ride. You hereby RELEASE from any and all liability Cycology Cycling Club’s officers, volunteers and agents, their respective successors and assigns, ride sponsors and supporters, and ride leader(s) (if any), against all claims, including, but not limited to, claims of negligence, unintentional acts, and acts of omission, and any personal injury, loss or expense, which may arise from your participation in any Cycology Cycling Club ride. You understand you are responsible for your own conduct and decisions while participating in any Cycology Cycling Club ride and further agree that this Waiver and release shall apply to any claim arising out of your participation in non-cycling activities while a participant in Cycology Cycling Club ride.

    This Waiver and release is intended to be binding upon you, your family, your heirs, your estate, your legal representatives, and assigns.
